In 2024 your school vaccination provider, Logan City Council, is moving to a simple digital system for our School Immunisation Program. This online system will replace the paper consent card previously used, making it quicker and easier to record if you like your child vaccinated at school.
How it works:
  • Use your phone, tablet or computer to follow the link to access the VaxApp parent portal: Windaroo Valley State High School
  • Register to create a VaxApp profile for your child and provide consent for the vaccines being given for their year level. Your child will then be booked in to receive their vaccine/s at their scheduled school visit date.
  • You can return to the portal at any time before the visit dates to confirm upcoming appointment dates or withdraw consent.
  • If you do not want your child to be vaccinated, you can select ‘I do not consent’ and prevent further follow-up from our team.
Electronic consent will be required for our upcoming visit on Friday 31 May 2024.

Please, follow the below link to register and submit digital consent by Tuesday 30April 2024.

Register now: Windaroo Valley State High School

Important Notes:
  • If you do not give consent for vaccinations at school, please still follow the link to submit the “I do not consent” electronically.
  • If you only want to consent for one of the two vaccinations available, then please ensure you select the “I consent” option. This will then allow you to de-select the vaccine you do not wish to provide consent for.
  • If your child is vaccinated at the school clinic on Friday 31 May 2024 you will be emailed the vaccination confirmation.
If you have any questions or require assistance to complete the online consent form, please contact Logan City Council’s Immunisation team on 3412 4039 or email immunisation@logan.qld.gov.au
